When recovering from an illness or injury, some people often need more care before returning home. Liberty Inn fills that gap by providing medical rehabilitation care and support prior to the person's transition back home.
Exceptional Service Delivery
Depending upon individual needs, a resident may receive physical, occupational and speech-language therapies, along with specialty services such as I.V. therapy, pain management, post-operative care, wound care, feeding tube monitoring, oxygen therapy and nutritional care. Our staff also places a priority on every resident's adjustment to illness or injury. For that reason, both the resident and family receive support and counseling services to make that adjustment as smooth as possible.
 An Expert Staff
Under the guidance of our Medical Director, our interdisciplinary team is dedicated to providing residents with the most compassionate and highest quality short-term rehabilitation care and support, to ensure that the transition from hospital to the home results in the best outcomes. Our staff includes physical, occupational and speech-language therapists, licensed nursing staff, social workers, activity staff, dietitian and rehabilitation and nursing paraprofessionals.
 Support
Residents and families are encouraged to participate in the care plan. Our professional social service staff provides confidential individual or family counseling and support. They will help make all the necessary arrangements to return home after a short-term illness or rehabilitation. We'll work with the resident, his or her family and physician to develop a plan for a smooth and safe transition back to home.

We take great care to provide a home-like environment for residents. Beautifully-appointed private and semi-private rooms with private bathrooms, spacious and inviting social and dining areas, a lovely gazebo and outdoor landscaping overlooking ponds and greenery, expansive areas for medical rehabilitation and activities, and an extensive list of amenities - even a beauty parlor -- go along way in making life at Liberty Inn a pleasant experience for residents and families.
Admissions and Discharge Information
 Liberty Inn is Medicare-Certified. For residents who may need home health services upon discharge, we can refer to a variety of home health agencies, including our own two agencies, Heritage Home Health, Inc. and American Eldercare, Inc. Our parent company, Heritage Park Retirement Communities, also offers full service independent and assisted living residences on our two beautiful campuses in Delray Beach for those who may need these services
